(1) Any member desiring information from his State association or desiring to communicate with other members of his State association may file with such State association an application therefor, in writing, which shall state: (a) His full name and address. (b) The amount of the participation value of his accounts in the State association at the time such application is made. (c) A statement specifying in detail the information he requests; or that he desires to communicate with the other members of the State association. If the application is to enable the member to communicate with other members, such application shall be accompanied by a complete copy of the proposed communication or communications. (d) The reasons for and the purposes of his requests. L.1963, c. 144, s. 118.
