(a) No person shall operate or assist in the operation of bingo games until such person has reached the age of eighteen, registered with the executive director on forms prescribed by him and has provided information which the executive director may reasonably require including, but not limited to, such person's criminal record, if any, moral character or business affiliations and thereafter has been assigned a personal identification number by him. Such personal identification number assignment may be revoked for cause. (b) Notwithstanding the provisions of subsection (a) of this section, the executive director may issue a personal identification number to a minor sixteen years of age or older, provided written permission from a parent or legal guardian of such minor is filed with the division.
