12–807.10. Payroll Adjustment Forms. A. Description: Forms to deduct money from employees’ wages. Information includes name, Social Security number, amount, description of deduction, payroll period beginning date and other related information. B. Retention: 3 years after termination of employment or cancellation of the authorization; destroy. HISTORY: Added by State Register Volume 18, Issue No. 6, eff June 24, 1994.
