Other business of applicant or control person

An applicant, licensee, or control person shall provide the information requested on the Form MU2 or Form MU4 regarding any business of the applicant, licensee, or control person other than the business for which a license is required under AS 06.60, including the nature of the business, the individual's position, title, or relationship with the other business, start date, hours per month, description of duties, and other information the department or the registry may request.
