(1) Within 60 days after the filing of a completed application for a license or license renewal to conduct bingo, the Department shall either issue a license or notify the applicant in writing, in accordance with ORS 183.310 to 183.550, that the license has been denied, and that the applicant is entitled to a hearing. The license shall be effective for one year from the date it is issued and may be renewed annually. The form of the license shall be prescribed by the Department and shall include: (a) The name of the licensed organization; (b) The class of license; (c) The expiration date of the license; (d) The authorized county and specific location where bingo games may be operated by the licensee; and (e) Any special conditions of the license. (2) Each license shall be conspicuously displayed by the licensee during operating hours at its authorized location.
