FORMAT

An EA is to be a concise document not to exceed 25 pages, containing the following: (1) A cover sheet including the name, address and telephone number of the person in the State Project Agency who can supply additional information. The cover sheet shall also contain: (a) designation of the statement as an EA; (b) title of the proposed activity that is the subject of the document; and (c) list of any involved cooperating agencies.
