Form of notice

A notice of appeal must include the following information: (1) the action by the division which the appellant is appealing and the relief the appellant seeks; (2) the grounds for the appeal; and (3) whether the appellant requests an informal conference with the division during the pendency of the appeal before the office of administrative hearings.
