Form Of Appeal

01. Form of Appeal. An appeal must be in writing on a form prescribed by the Racing Commission and include: (7-1-24) a. The name, address, telephone number and signature of the licensee making the appeal; and (7-1-24) b. A statement of the basis for the appeal. (7-1-24) 02. Bond. The licensee filing the appeal may be required to furnish a bond in the amount of two hundred dollars ($200) to cover the administrative costs and which may be forfeited should the appeal be heard.
