A. Persons desiring to obtain a certificate as an approved plant breeder shall make application to the commission on a form supplied by the state entomologist. B. Applicants must qualify under the eligibility requirements set forth in §907. C. Applicants for a certificate qualifying under Paragraphs 1, 2, 3 and 4 of §907 shall supply with their application a copy of his college or university transcript; and in the cases of Paragraphs 2, 3 and 4, affidavits supporting the experience requirements indicated under Paragraph 2; and under Paragraph 5, supply affidavits and/or other documentary evidence supporting the requirements set forth herein.
