(1) A licensee shall notify the director of any of the following changes in the licensed business: (a) any change of business structure; (b) material change of ownership; (c) voluntary or involuntary business closure; (d) changes in the officers; (e) changes in the board of directors; (f) change of business address; and (g) any change of bank account information. (2) Pursuant to the licensee's contract, a licensee shall notify the director of any change, move, or removal of lottery terminal, equipment, and/or peripherals.
