Applications

01. Application Forms. All applications must be completely and legibly filled out and submitted to the Racing Commission on forms obtained from the Racing Commission, and all persons applying for licenses shall submit completed applications meeting all requirements, including obtaining necessary signatures as indicated on the form or otherwise noted in this chapter. License types are listed in the License Fee section of this chapter. (7-1-24) 02. Other Forms. All other forms to be submitted to the Racing Commission by this chapter must be of a type approved by the Racing Commission. (7-1-24) 03. Age. Applicants between sixteen (16) and eighteen (18) years of age are required to submit to the Racing Commission an Assumption of Liability Form signed by their guardian and notarized by a notary public.
