The contingency plan shall be reviewed and immediately amended and resubmitted to the Board, if necessary, whenever: (1) the plan fails in an emergency; (2) the facility changes -- in its design, construction, operation, or maintenance; (3) the list of alternate changes; or (4) the list of emergency equipment changes.
