When a claim is transferred pursuant to Bankruptcy Rule 3001(e)(2) or (e)(4), the notice of transfer of claim shall include a reference to the claim number of the original Proof of Claim for which the transferee shall be substituted for the transferor and the amount of the claim and shall be ﬁled on Administrative Office of the U.S. Courts Director’s Procedural Form 2100A Transfer of Claim Other Than for Security.
