(a) Confirmation Affidavit. Before the 341 Meeting and no later than prior to confirmation of a chapter 13 plan, the debtor must execute an affidavit in the form of LBF 3015-2A and file it with the court. (b) Discharge Affidavit. Prior to the issuance of a chapter 13 discharge, the debtor must execute an affidavit in the form of LBF 3015-2B and file it with the court.
