Notation of “Jury Demand” in the Pleading

(a) If a party demands a jury trial as Fed. R. Civ. P. 38(b) permits, the party shall place a notation on the front page of the initial pleading which that party signed , stating “Demand for Jury Trial” or an equivalent statement. This notation shall serve as a sufficient demand under Fed. R. Civ. P. 38(b). (b) In cases removed from state court, a party may file a “Demand for Jury Trial” that is separate from the initial pleading. See Fed. R. Civ. P. 81(c); L.R. 81.3.
