Motions and all subsequently filed documents related to a motion shall state below the title of the document (a) the date and time of the hearing, when known, (b) the name of the presiding judge, (c) the trial date, if known, and (d) the docket number of the document to which it relates (i.e., an opposition memorandum shall indicate the CM/ECF document number of the relevant motion). 8
