(a) Non-governmental Corporate Party. Unless otherwise ordered by the Court, any party required by Fed. R. Crim. P. 12.4(a)(1) to file a disclosure statement must do so using the “Fed. R. Crim. P. 12.4 Disclosure Statement” form located on the Court’s website. (b) Organizational Victim. Unless otherwise ordered by the Court, the government must file the disclosure statement required by Fed. R. Crim. P. 12.4(a)(2) using the “Fed. R. Civ. P. 12.4 Disclosure Statement” form located on the Court’s website.
