Any pleading asserting a claim by or against a class shall: (a) bear, next to the caption, the designation “Class Action”; and (b) contain a section entitled “Class Action Allegations” immediately after the allegation of jurisdictional grounds, which section shall contain: (1) (2) a reference to the specific provision(s) in Rule 23(b) of the Federal Rules of Civil Procedure under which it is claimed that the action may be brought as a class action; and sufficient allegations to support the claim that the action may be brought as a class action, including but not limited to: (A) the number or approximate number of the members of the class; (B) a description of who is in the class; (C) the basis upon which it is claimed that the party asserting the claim will fairly and adequately protect the interest of the class, or, if the claim is asserted against a class, that the named members of the class will fairly and adequately protect the interests of that class; and (D) the questions of law or fact claimed to be common to the class.
