A complaint or other pleading asserting a class action shall contain sufficient allegations to identify the class and the claim as a class action, including, but not limited to: (a) The approximate size and definition of the alleged class; (b) The basis upon which the party or parties maintaining the class action or other parties claimed to be representing the class are alleged to be adequate representatives of the class; (c) The alleged questions of law and fact claimed to be common to the class; (d) The grounds upon which it is alleged that the claims or defenses of the representative parties are typical of the claims or defenses of the class; and (e) Allegations intended to support findings required by the respective subsections of Fed. R. Civ. P. 23(b)(1), (2), or (3).
