Civil Cover Sheet and Complaint

Every complaint or other document initiating a civil action shall be accompanied by a completed civil cover sheet Form JS-44, which is available from the Court Clerk's office, or from the Court's website at www.oked.uscourts.gov. Counsel and prose litigants are required to 4 number each party separately in the caption of the complaint, plaintiffs consecutively and defendants consecutively. The face of the Complaint shall state whether the action is related to any previously filed case in this Court and identify by number said related case.
