• A civil cover sheet form, available from the Clerk, must be completed and attached to the pleading initiating a civil action. This requirement is for administrative purposes, and information appearing on the civil cover sheet will have no legal effect in the action. • The Clerk will file the initiating paper even if it is submitted without the completed cover sheet. The Clerk will give written notice of the deficiency and deadline for correcting the deficiency. The Court may dismiss the action, without prejudice, for failure to cure the deficiency. • A pro se party filing a civil action need not complete the cover sheet.
