(a) Title of Plan and Disclosure Statements. Every plan and disclosure statement filed in a chapter 11 case and any modified plan or disclosure statement shall include the date of the paper and the identity of the plan proponent in the title. (b) Filing. A disclosure statement shall be accompanied by (1) the plan and (2) a notice setting the hearing date and the deadline for objecting to the disclosure statement.

(c) Plan to be Filed as Separate Document. Any plan shall be filed as a separate document and not be attached as an exhibit to the disclosure statement.
