After the approval of the disclosure statement, the plan proponent shall file a copy of the disclosure statement and a proposed order containing the following: (a) A statement that the disclosure statement was approved under § 1125(b) of the Code; (b) The deadline for serving the order, disclosure statement, plan, and ballot on interested parties; (c) The deadline for voting to accept or reject the plan; (d) The deadline for filing objections to the plan; (e) The date of the confirmation hearing; and (f) A statement that within seven days after mailing the plan and disclosure statement as required by this order, the debtor shall file a certificate of service along with copies of each 11 document mailed.
