(a) An attorney appearing for a party in a civil case shall either (a) sign the initial pleading filed on behalf of that party or (b) sign and file an entry of appearance on the form prescribed by the Court Clerk. (b) In the event a party should change counsel or add additional counsel, the new or additional counsel for such party shall enter an appearance by signing and filing an entry of appearance on the form prescribed by the Court Clerk. (c) In the event an attorney is appearing Pro Hac Vice, that attorney shall file an Entry of Appearance and register for electronic case filing within seven (7) days after granting of the Motion for Pro Hac Vice.
