(a) Entry of Appearance. An attorney appearing for a party must enter an appearance by signing and filing an entry of appearance in the case, using Form AT-04, Attorney Appearance, available on the Court’s website. (b) Withdrawal. Attorneys of record must not withdraw from the case except upon reasonable notice to the client and all other parties who have appeared in the case and by leave of the judge to whom the case is assigned.
