(A) All petitions to review, petitions to enforce, and petitions to adjudge in contempt, shall be accompanied by a legible copy of the appropriate agreement, order, and/or decree sought to be reviewed or enforced. The court shall not accept any petition which is not filed in accordance with this rule. In the event any such petition is accepted by the court, it shall be subject to being dismissed without prejudice for failure to be in proper form. (B) All petitions seeking payment of bills for medical services rendered shall be accompanied by a legible copy of the bill, the report upon which the bill for services is based and, if liability has been established, a legible copy of the agreement, order, and/or decree sought to be reviewed. All such petitions shall also be accompanied by an affidavit of a treating or consulting doctor that the service provided was necessary to cure, rehabilitate or relieve the person to whom the service was rendered of the effects of the injury set forth in the agreement, order, and/or decree under review. The court shall not accept any petition which is not filed in accordance with this rule. In the event any such petition is accepted by the court, it shall be subject to dismissal without prejudice for failure to be in proper form.
