Pagination and Indexing of Supplements

(A) Pagination 93 The pages of the supplement shall be consecutively numbered in the bottom right-hand corner. (B) Transcripts If any portion of a transcript is included in the supplement, the original page numbering of the transcript shall be placed in parentheses. (C) Index The supplement shall include an index that lists all items included in the supplement and references the page numbers at which each item can be located.
