General Requirements of Form

(a) LEGIBILITY; ABBREVIATIONS. A petition, pleading, schedule, or other document must be clearly legible. Commonly used English abbreviations are acceptable. (b) CAPTION. A document presented for filing must contain a cap- tion that sets forth: (1) the court’s name; (2) the case’s title; (3) the case number and, if appropriate, adversary-proceed- ing number; and (4) a brief designation of the document’s character.
