(A) If after the preliminary investigation has been completed, the commission concludes that a formal hearing shall be held, the cause shall be entered in a docket to be kept for that purpose. The complaint shall be entitled: "Before the Commission on Judicial Qualifications, complaint against __________, judge, No. __________." (B) A complaint shall be in a form similar to a complaint filed in a civil action in the district court. It shall be filed in the office of the Clerk of the Supreme Court, there to become a public record, and a copy shall be promptly served on the respondent, either by certified mail or personally. If the respondent cannot be found in the State of Nebraska, service may be had by leaving a copy at respondent's last known Nebraska address.
