Form of Oath, Award and Notice of Award
Illinois Supreme Court Rules
The oath, award of arbitrators, and notice of award shall be in substantially the same form as the template provided in the Article I Forms Appendix. Adopted May 20, 1987, effective June 1, 1987; amended March 1, 2001, effective immediately; amended October 20, 2003, effective December 1, 2003; amended June 22, 2017, eff. July 1, 2017.
