Every Equity Petition shall contain the county, title of the Court, names, places of abode, and proper description of all Parties to the proceeding. The Petition shall include the complete mailing and service addresses for the petitioner(s) and all respondents. The form in substance shall be as follows: County of _______________ Month, Year A.B. v. C.D. NAME OF PETITION A.B. of etc., complains against C.D. of etc., and says, etc. and requests, etc... The Petition may conclude "and thereupon the Petitioner prays," setting forth the special relief sought, "and for such other relief as may be just". If an injunction or other special order pending the suit is desired, it shall be specifically requested.
