Actions (a) Pursuant to the General Statutes, the judg- ment creditor or the representative of the judg- ment creditor may file with the court a written application on forms prescribed by the Office of the Chief Court Administrator for an execution to collect an unsatisfied money judgment. (b) Service of an initial set of interrogatories, on forms prescribed by the Office of the Chief Court Administrator relevant to obtaining satisfac- tion of a small claims money judgment shall be made by sending the interrogatories by certified mail, with return receipt requested or with elec- tronic delivery confirmation, to the person from whom discovery is sought.
