section 229.14 must state with reasonable particularity on the form prescribed by this chapter the facts and basis for the diagnostic conclusions concerning the respondent’s serious mental impairment and recommended treatment, including but not limited to: The basis for the chief medical officer’s conclusion as to respondent’s mental illness, judgmental capacity concerning need for treatment, treatability, and dangerousness; and the basis for the chief medical officer’s conclusions concerning recommended treatment including the basis for the judgment that the chief medical officer’s treatment recommendation is the least restrictive alternative treatment pursuant to options (a), (b), (c), or (d) of Iowa Code section 229.14(1).
