The complaint in an action before a magisterial district judge to which an incapacitated person is a party shall be entitled in the name of the incapacitated person, followed by the phrase ‘‘an incapacitated person, represented by A, Guardian.’’ The address of the incapacitated person and that of the guardian shall be shown on the complaint form. Official Note Since the rules in this chapter relating to incapacitated persons deal with persons who already have guardians, the complaint is entitled to show that the incapacitated person, whether plaintiff or defendant, is represented by a guardian.
