(A) The notice shall state which method of review is being sought in the court of common pleas by indicating whether it is a notice of appeal or notice of a petition for a writ of certiorari. (B) The notice shall contain the following information: (1) The name and address of the defendant. (2) The date of imposition of sentence. (3) The charges upon which the defendant was convicted. (4) The name of the sentencing judge. Comment The notice form previously set forth in this rule was deleted in 1996 because it is no longer necessary to control the specific form by rule.
