A civil action is commenced by filing a complaint with the court. Upon filing such a complaint, the filing party must complete a civil cover sheet provided by the justice court, and approved by the state court administrator, that contains certain information regarding the nature of the action being filed. This cover sheet must be signed by the initiating party, or his or her representative, and the filing may be rejected if unaccompanied by such a cover sheet. [Amended; effective October 29, 2024.]
