Certificates of service

The last page of every paper served in accordance with Mass.R.Civ.P. 5(a) or Mass.R.Dom.Rel.P. 5(a) shall contain a brief statement showing the date on which and manner in which service of the paper was made on each other party, with the name and address of each person served. The statement may be in the following form: I hereby certify under the penalties of perjury that a true copy of the above document was served upon the following people at the following addresses by mail (by hand) on (date): (Names and addresses), (Signature).
