(a) CAPTION AND TITLE; REQUIRED INFORMATION. A petition’s cap- tion must contain the name of the court, the title of the case, and the case number (if known). The title must include the following information about the debtor: (1) name; (2) employer-identification number; (3) the last 4 digits of the social-security number or individ- ual taxpayer-identification number; (4) any other federal taxpayer-identification number; and (5) all other names the debtor has used within 8 years before the petition was filed. (b) PETITION NOT FILED BY THE DEBTOR. A petition not filed by the debtor must include all names that the petitioner knows have been used by the debtor. (As amended Mar. 30, 1987, eff.
