I-693, Report of Immigration Medical Examination and Vaccination Record Alert Type info Alert: We now offer online filing of Form I-485, Application to Register Permanent Residence or Adjust Status , for certain employment-based applicants and their representatives. We are in the process of developing the online filing capability for other applicant types. When you file Form I-485, you must also submit Form I-693, Report of Immigration Medical Examination and Vaccination Record . A civil surgeon is responsible for providing you with a completed Form I-693 that is signed and placed in a sealed envelope.

If you file Form I-485 online, you must open the sealed envelope from the civil surgeon and upload the completed Form I-693 with your application package. You must keep the original Form I-693 and the envelope from the civil surgeon until we make a final decision on your Form I-485. We may review the original Form I-693 and envelope during an interview or ask you to submit the original Form I-693 and envelope as evidence to support your application. If you file Form I-485 by mail, there are no changes to the Form I-693 submission process.

You must submit Form I-693 with your Form I-485 application package, in the original sealed envelope provided by the civil surgeon. Alert Type info ALERT: The USCIS lockbox facility may reject an application package for various reasons, including failure to pay the application fee or sign the application. If we reject your application and return your application package with the Form I-693 envelope opened, and you wish to file a new application package with the original opened Form I-693, you may include the original opened Form I-693 and any supporting documentation that was previously included by the civil surgeon in its original envelope with a copy of the rejection notice. If you are applying to adjust your status to lawful permanent resident (also known as getting a Green Card), use Form I-693 to establish that you are not inadmissible to the United States on health-related grounds.

Effective Dec. 2, 2024, you must submit Form I-693, or a partial Form I-693 (such as the Vaccination Record), and you must submit it with your Form I-485. Otherwise, we may reject your Form I-485. The following aliens applying for adjustment of status must submit a partial Form I-693 (Parts 1.-5., Part 7, and Part 10, Vaccination Record) when they file their adjustment application: Refugees (Form I-590 or Form I-730) who already completed an immigration medical examination conducted by a panel physician outside the United States; or Derivative asylees (Form I-730) who already completed an immigration medical examination conducted by a panel physician outside the United States and applied for adjustment of status within 1 year of eligibility to file.

The following aliens applying for adjustment of status must submit a partial Form I-693 (Parts 1.-5., Part 7, and Part 10, Vaccination Record) when they file their adjustment application if their vaccination record was not properly completed and included as part of the immigration medical examination conducted by a panel physician outside the United States: Aliens admitted to the United States as a spouse, or child of a spouse, of a U.S. citizen or fiancé(e) or child of a fiancé(e) of a U.S. citizen (K-1/K-2/K-3/K-4 nonimmigrant) who already completed an immigration medical examination conducted by a panel physician outside the United States no more than 1 year before they file to adjust their status; or Afghan nationals who arrived in the United States under Operation Allies Welcome who already completed an immigration medical examination conducted by a panel physician outside the United States. NOTE: If you received an immigration medical examination outside the United States and the results reveal a Class A medical condition, or if there is reason to believe you have acquired a Class A medical condition after admission or parole into the United States, you may need to complete a new immigration medical examination. You should review the Form I-693 Instructions to determine what information on Form I-693 you need to complete and submit to USCIS, if required. You can find a list of the health-related grounds of inadmissibility in section 212(a)(1) of the Immigration and Nationality Act.

Where to File After completing your immigration medical examination, the civil surgeon must give you the completed and signed Form I-693 in a sealed envelope. Do not accept the Form I-693 if it is not in a sealed envelope. You (not the civil surgeon) must submit your Form I-693 to USCIS.

Submitting Your Form I-693 If You File Your Adjustment of Status Application With USCIS by Mail If you are applying for adjustment of status to lawful permanent resident (a Green Card) by mail, you must submit Form I-693 by mail with your Form I-485, to the location specified for your Form I-485 (see the Direct Filing Addresses for Form I-485 ). Otherwise, we may reject your Form I-485. Submitting Your Form I-693 If You File Your Adjustment of Status Application With USCIS Online If you file your Form I-485 online, you must open the sealed envelope from the civil surgeon and upload the completed Form I-693 with your Form I-485 application package. You must keep the original Form I-693 and the envelope from the civil surgeon until we make a final decision on your Form I-485.

Other aliens: applying for adjustment of status with EOIR or other aliens required to provide an immigration medical examination on Form I-693: Follow the instructions on or included with the application, or the instructions given to you by the office requesting the immigration medical examination. When to File If you are filing Form I-485, and if you are required to submit a Form I-693 or partial Form I-693 (such as the Vaccination Record), you must submit it with your Form I-485. Otherwise, we may reject your Form I-485.

Special Instructions eMedical The Centers for Disease Control and Prevention (CDC) updated the Tuberculosis Technical Instructions for Civil Surgeons . Civil surgeons must register for access to eMedical, an electronic health processing system.

Using eMedical will help the CDC maintain public health security and transition to electronic submission of immigration medical examinations. Civil surgeons should have received emailed instructions to register; it is important that you follow these instructions and register immediately .
