8 FAM 1302 Defective Passports, Foils, and BarCodes for Posts .1 Defective Passports, Foils, and Bar Codes for Posts (CT:CITZ-56; 05-21-2021) (Office of Origin: CA/PPT/S/A) .1-1 Defective Passports (CT:CITZ-1; 06-27-2018) See 8 FAM 1001.5 for guidance on replacement of defective passports. .1-2 Supplemental Passport Foils (CT:CITZ-56; 05-21-2021) a. Supplemental passport foils are controlled accountable items; they must be inventoried and used correctly. Do not use defective or duplicate supplemental passport foils. See 7 FAH-1 H-652.1 for definitions of controlled and accountable items . b. If a supplemental passport foil is found to be defective, report the problem(s) on form DS-859 and return the defective items with form DS-859 via classified pouch. c. If the defect is discovered after form DS-859 has been sent, report the defect to CA/EX/GSD by e-mail at [email protected] and await instructions regarding disposition. d. If the defect is discovered during production, do not use the supplemental passport foil. Type the number of the next available supplemental passport foil into the passport service in American Citizens Services and apply a non-defective supplemental passport foil to the passport application you are processing. .1-3 Barcodes (CT:CITZ-56; 05-21-2021) a. Barcodes are controlled , limited -accountable items. See 7 FAH-1 H-652.1 for definitions of controlled and accountable items. They must be inventoried and used correctly. Do not use defective or duplicate barcodes; b. If a barcode is found to be defective, report the problem(s) on form DS-859, and return the defective items with form DS-859 via classified pouch; c. If the defect is discovered after form DS-859, has been sent, report the defect to CA/EX/GSD by e-mail at [email protected] and await instructions regarding disposition; and d. If the defect is discovered during production, do not use the barcode. Type the number of the next available non-defective barcode into the passport service in ACS and apply the new barcode sticker to the passport application you are processing.
